Caregivers skip their own appointments, sleep badly, and describe exhaustion as laziness. Let them answer the doctor's questions first, then fill in gaps of recall rather than retelling their story for them. Caregiving is unpaid, unscheduled, and mostly invisible, and it reliably does three things to the person doing it: it interrupts sleep, it postpones their own appointments, and it turns exhaustion into something they describe as laziness. None of that is a character flaw.
