Can Anxiety Cause High Blood Pressure?¿La ansiedad puede subir la presión arterial?

The short answer: yes, in the moment. That is exactly why a reading taken while you're anxious can't tell you much about your blood pressure. When anxiety hits, your body releases adrenaline. Your heart beats faster and harder and your blood vessels tighten, so the pressure rises within minutes and falls again once you calm down. Whether anxiety causes lasting high blood pressure is less settled. Mayo Clinic says it doesn't, but frequent daily spikes can do the same damage. A 2021 review of 59 studies and more than 4 million people found that people with anxiety had about 40% higher odds of hypertension. What helps: judge your pressure by a calm, scheduled average, and treat the anxiety as a real problem in its own right.

This article explains what anxiety does to the number, how the "checking loop" keeps people stuck, what the research actually shows, and how to get a reading you can trust.

I hear this question in two very different voices. One person was told at a clinic, "It's probably just nerves," and wants to know if they can safely ignore it. Another person is so frightened by one high number that they now check ten times a day, and every check comes back higher. In patient forums people describe it plainly: the cuff starts to inflate, the heart starts to pound, and the number on the screen confirms the fear. Is this real, or is it me?Escucho esta pregunta en dos voces muy distintas.

Both people deserve a straight answer. Here it is.Las dos personas merecen una respuesta directa. Aquí está.

What anxiety does to the reading in the momentLo que la ansiedad le hace a la lectura en el momento

Anxiety switches on your body's stress response, the same system that would help you run from danger. Stress hormones such as adrenaline make the heart pump faster and harder and squeeze the small arteries narrower. More blood pushed into narrower pipes means higher pressure. It happens within minutes, and when the fear passes the pressure comes back down.

The cuff has no way of knowing why your pressure is up. It measures what is in the artery at that instant. So if you are afraid of the reading itself, because of a scary visit, a stern warning or a bad night in an emergency room, the reading partly measures that fear. This is one reason the 2025 AHA/ACC guideline recommends confirming a diagnosis with readings taken outside the clinic, at home or with a 24-hour monitor, and judging blood pressure by the average of several careful readings, never by one.

The checking loop, and how to break itEl ciclo de medirse, y cómo romperlo

Here is the trap many people fall into once they are worried about their pressure:Esta es la trampa en la que caen muchas personas cuando ya están preocupadas por su presión:

You feel anxious Se siente ansioso You check your pressure Se toma la presión Adrenaline → higher number Adrenalina → número más alto The number scares you El número lo asusta The checking loop El ciclo de medirse each check measures the fear cada lectura mide el miedo
Checking because you feel anxious produces the number most likely to make you more anxious. A schedule breaks the loop; willpower rarely does.

The way out is not to stop measuring. It is to take the decision about when to measure away from the feeling:La salida no es dejar de medirse.

  1. Measure on a schedule, not on a feeling. The American Heart Association technique is two readings one minute apart, morning and evening, on the days your clinician asks for. Outside those times, the cuff stays in the drawer.Mídase con horario, no por lo que siente.
  2. Use the calm routine every time. No caffeine, exercise or smoking for 30 minutes before. Empty your bladder. Sit quietly for five minutes, back supported, feet flat, legs uncrossed, arm resting at heart level, cuff on a bare arm. No talking and no phone.
  3. Don't chase a high number. Take your two readings, write both down, and stop. A third, fourth and fifth reading taken because the first one scared you will not calm you down, and it won't give your doctor better information. The one exception is a reading of 180/120 or higher: then follow the red box below.
  4. Write down how you felt. "Just argued with my son" next to a reading is useful information for your clinician. A pattern of high readings only on bad days tells a different story than high readings on calm mornings.
  5. Use a validated upper-arm cuff. Look it up at validatebp.org. Smartwatches and rings that claim to read blood pressure without a cuff are not for diagnosis.

If home measuring itself has become the trigger, say so. It's a common problem, and your clinician has options. A 24-hour ambulatory monitor is a guideline-recognized way to confirm a diagnosis. It takes many readings through the day and night, including while you sleep, so one frightened moment gets averaged in with everything else instead of deciding the answer. The first few inflations can feel stressful. That's expected, and the average still works.

Does anxiety cause long-term high blood pressure? Where the evidence stands¿La ansiedad causa presión alta a largo plazo? Lo que dice la evidencia

This is where you will read two answers that sound like they contradict each other.

These two statements can both be true. A link between anxiety and hypertension doesn't prove that anxiety itself pushes the pressure up for good. The habits that often travel with anxiety, the medicines, poor sleep, and anxious readings in the clinic could all explain part of it. What both sources agree on is the practical part: frequent spikes are not harmless, and anxious people are not protected from real hypertension.

"It's just anxiety" and "my anxiety is destroying my arteries" make the same mistake: they let one scared reading speak for your whole day.«Son solo nervios» y «mi ansiedad me está destruyendo las arterias» cometen el mismo error: dejan que una lectura asustada hable por todo su día.

The mistake in both directionsEl error en las dos direcciones

🚨 Call 911 (or your local emergency number) right away if:🚨 Llame al 911 (o al número de emergencias de su localidad) de inmediato si:

  • Your reading is 180 or higher on top and/or 120 or higher on the bottom and you have chest pain or pressure, shortness of breath, back pain, numbness or weakness, trouble speaking, vision changes, a sudden severe headache, or confusion.
  • You have chest pain or pressure or trouble breathing, whatever the number says, even if you have had panic attacks before. A panic attack and a heart attack can feel alike, with a pounding heart, sweating and shortness of breath. You cannot tell them apart at home, and neither can a blood pressure cuff. Let the emergency team decide.

That high, but no symptoms? Sit quietly for 5 minutes and measure again. If it is still 180/120 or higher, contact your clinician the same day (urgent care if you can't reach them). Don't take an extra dose of anything on your own.¿Así de alta, pero sin síntomas? Siéntese tranquilo 5 minutos y vuelva a medir.

Anxiety that makes daily life hard deserves care in its own right, not only because of what it does to a cuff reading. Tell your clinician how much it is affecting you. A mental health professional can help you find the right treatment. The 2025 blood pressure guideline also lists stress management, such as meditation, breathing practices and yoga, among its everyday lifestyle steps, alongside less sodium, less alcohol, regular activity and weight loss if needed.

If you already take a medicine for anxiety or mood and your readings have changed, bring that up. Never stop or change an anxiety or blood pressure medicine on your own. Stopping some of them suddenly causes problems of its own.

What to say at your next visitQué decir en su próxima consulta

"My readings jump when I'm anxious, and checking has turned into a habit I can't stop. Here's my log from scheduled morning and evening readings, with notes on how I felt. What is my real average, and does it count as high blood pressure? Would a 24-hour monitor help settle it? And can we talk about the anxiety itself, not just the numbers?"«Mis lecturas se disparan cuando estoy ansioso, y medirme se volvió un hábito que no puedo parar.

If you take a medicine for anxiety or mood: "Could any of my medicines be raising my pressure?"Si toma un medicamento para la ansiedad o el ánimo: «¿Alguno de mis medicamentos podría estar subiéndome la presión?»

Here is my read, and it is my opinion. A reading you take while you're scared isn't a wrong reading. It's an honest answer to a different question. It tells you what your pressure does when you're afraid, and that is not the number anyone uses to diagnose you. So I'd stop asking "is this number real?" and start asking "which number am I looking at?" The calm, scheduled average answers the question your doctor is asking. The 3 a.m. reading after a bad dream answers a question nobody asked.

The other half: I would treat the anxiety even if it did nothing to blood pressure. The 2021 meta-analysis is one more reason, not the main one. Someone who checks their pressure fifteen times a day is suffering, whatever the cuff says, and that deserves care as much as any number on the screen.

Common questionsPreguntas frecuentes

Can anxiety cause high blood pressure?¿La ansiedad puede causar presión alta?

In the moment, yes. Anxiety triggers the body's stress response: adrenaline makes the heart beat faster and harder and narrows blood vessels, so blood pressure rises within minutes and falls again once you calm down. Whether anxiety causes lasting high blood pressure is less settled. Mayo Clinic states that anxiety does not cause long-term high blood pressure, but that temporary rises happening often, such as every day, can damage the blood vessels, heart and kidneys in the same way long-term hypertension does. A 2021 meta-analysis of 59 studies with more than 4 million participants found that people with anxiety had higher odds of hypertension, including in studies that followed people forward in time, though an association like this does not prove that anxiety itself is the cause.

Should I stop checking my blood pressure at home if it makes me anxious?¿Debo dejar de medirme la presión en casa si me pone ansioso?

Usually the answer is not to stop, but to change how you check. Measure on a schedule rather than when you feel worried: morning and evening, two readings one minute apart, on the days your clinician asks for, using the calm routine (no caffeine, exercise or smoking for 30 minutes, empty bladder, five minutes of quiet sitting, back supported, feet flat, arm supported at heart level, cuff on a bare arm, no talking). Write both readings down and do not chase a high one with reading after reading. If home measuring itself has become the trigger, tell your clinician. A 24-hour ambulatory monitor, which takes many readings through the day and night, is a recognized alternative for confirming a diagnosis.

How do I know if my high readings are anxiety or real high blood pressure?¿Cómo sé si mis lecturas altas son ansiedad o presión alta de verdad?

You cannot tell from a single reading, and neither can your doctor. The 2025 AHA/ACC guideline recommends confirming a diagnosis with readings taken outside the office, either at home or with a 24-hour ambulatory monitor, and classifying blood pressure by the average of several properly measured readings. If your calm, scheduled average runs 130/80 or higher, that counts as high blood pressure whether or not you are an anxious person, and it deserves a plan with your clinician. If your readings are high only in the clinic or only when you are upset, that pattern is also worth reporting, because it changes how your clinician interprets the numbers.

Some can. Mayo Clinic notes that certain medicines used for anxiety and other mental health conditions, including serotonin and norepinephrine reuptake inhibitors (SNRIs), can raise blood pressure. Some over-the-counter products, such as decongestants, can too. If you take one of these and your readings have changed, bring your full medication list to your prescriber. Never stop or change an anxiety or blood pressure medicine on your own, because stopping some of them suddenly can cause problems of its own.
