The short answer: your blood pressure is not supposed to hold one level all day. In most people it falls by roughly 10 to 20 percent while they sleep — the longest stretch of the day when the pressure inside your arteries comes down. That is why, in 2022, the American Heart Association added sleep to its heart-health checklist for the first time, making it the eighth item. And in April 2025 the AHA went further: the thing nearly all of us track, hours, is only one dial of several. How consistent your sleep is may matter as much as how long it is. What to do: protect a steady wake time first, aim for 7 to 9 hours, and if you snore or wake up unrefreshed, get that looked at.La respuesta corta: su presión arterial no debe mantener un solo nivel todo el día.

This article explains what your blood pressure actually does overnight, why the heart checklist grew from seven items to eight, where the evidence is softer than the headlines suggest, and what is genuinely worth changing this week.

Dennis had done everything on the list. He is 52, walks the dog twice a day, cut the salt after his father's stroke, lost eleven pounds, and takes his blood pressure at home like a good student. His afternoon readings were fine. His morning readings were creeping up. His doctor had checked his kidneys, his thyroid and his salt habits, and nothing explained it.Dennis había hecho todo lo de la lista.

Nobody had asked about the eight hours in between. Not whether he slept — he did, most nights, more or less. What nobody had asked was when. Dennis went to bed at 10 on Monday, at 1 a.m. on Wednesday when work ran long, and at 2 on Friday, then slept until 11 on Saturday to "catch up." On paper, he averaged close to seven hours. On the calendar, his body started a new time zone every three days.

(Dennis is a composite — a blend of many people I have cared for, not one patient. The pattern, though, is extremely common.)(Dennis es un caso compuesto — una mezcla de muchas personas que he atendido, no un paciente en particular. El patrón, sin embargo, es muy común.)

What your blood pressure does while you are asleepQué hace su presión arterial mientras usted duerme

Here is the part almost nobody is taught. Blood pressure has a daily rhythm. It climbs in the morning, runs at its working level through the day, and then — in most people — falls during sleep, usually by about 10 to 20 percent. Researchers call that fall nocturnal dipping, and a person whose pressure behaves that way is called a "dipper."Esta es la parte que casi nadie aprende.

If the fall is less than 10 percent, that is called non-dipping. If the pressure actually goes up overnight, that is reverse dipping. None of this is a moral report card — it is physiology, and it is measured, not felt.

The night shift your arteries are supposed to work El turno de noche que sus arterias deberían trabajar Top (systolic) number across one day and night Número de arriba (sistólico) a lo largo de un día y una noche ASLEEP DORMIDO 130 120 110 mm Hg 6 AM 12 PM 6 PM 12 AM 6 AM Typical: the pressure dips 10–20% overnight Típico: la presión baja 10–20% de noche Blunted: it barely comes down Aplanado: casi no baja Illustration of the pattern — not real patient data. Only a 24-hour monitor ordered by your clinician shows your own night.
Blood pressure is a rhythm, not a number. Sleep is the only long stretch of the day when it is meant to come down.
Your body doesn't just rest at night. It lowers the pressure.Su cuerpo no solo descansa de noche. Baja la presión.

Here is the catch, and it is a practical one: your home cuff cannot see any of this. A home monitor only records the moments you are awake and using it. The test that actually measures sleeping blood pressure is 24-hour ambulatory monitoring — a cuff you wear for a full day and night that inflates on its own, including while you sleep. It is something a clinician orders when the question matters, not something to go buy out of curiosity.

The year the heart checklist grew from 7 items to 8El año en que la lista del corazón pasó de 7 puntos a 8

For years the American Heart Association's shorthand for heart health was "Life's Simple 7": diet, activity, smoking, weight, cholesterol, blood sugar, blood pressure. In June 2022, it became Life's Essential 8. The new eighth item was sleep duration, and the stated ideal for adults is 7 to 9 hours a day.

That is a bigger deal than it sounds. A checklist like that is what shapes what your doctor asks you, what gets measured at a health fair, and what an employer's wellness program tracks. For decades, sleep was simply not on it. The CDC's recommendation runs in the same direction: 7 or more hours for adults aged 18 to 60, and it links short sleep to type 2 diabetes, heart disease, high blood pressure and stroke. The NHLBI puts it plainly: sleep deficiency "is linked to many chronic health problems, including heart disease, kidney disease, high blood pressure, diabetes, stroke, obesity, and depression."Eso es más importante de lo que parece.

Then 2025 complicated it — in a useful wayLuego 2025 lo complicó — de una manera útil

On April 14, 2025, the American Heart Association published a scientific statement in Circulation: Cardiovascular Quality and Outcomes making a point that has not yet reached most waiting rooms: healthy sleep is not one measurement.

And then the finding that should change how we talk about this. According to that statement, greater consistency in sleep-wake timing has been associated with a 22 to 57 percent lower risk of cardiovascular death, and irregular sleep timing was linked to a higher risk of type 2 diabetes even in people who slept enough hours. A 2024 study that put accelerometers on 60,977 adults — more than 10 million hours of recorded sleep — reported that sleep regularity was a stronger predictor of mortality risk than sleep duration, with the more regular sleepers showing roughly 20 to 48 percent lower all-cause mortality than the least regular group.Y luego el hallazgo que debería cambiar cómo hablamos de esto.

My perspective as a physicianMi perspectiva como médico

Here is a thought worth sitting with, offered as perspective rather than settled guideline. We built the checklist around the measurement that was easiest to collect: hours. That is understandable — you can ask "how long do you sleep?" on any form. But the body does not run on a total; it runs on a clock. Every cell in your blood vessels keeps time, and a schedule that lurches by three hours every few days asks that clock to keep restarting. My read of the newer evidence is that the checklist captured the right topic with the wrong single number, and that regularity will keep rising in importance as it becomes easier to measure. What I would not do is turn that into a new thing to fail at. If your schedule is genuinely out of your control — night shifts, a newborn, two jobs — the answer is not guilt. It is to protect whatever anchor you can hold, and to tell your doctor the truth about your schedule so the rest of your care is planned around the life you actually have.

Where the evidence is softer than the headlinesDónde la evidencia es más débil que los titulares

I want to be straight with you, because a lot of sleep content online is not. You will read that "non-dippers" have worse outcomes, stated as flatly as gravity. The physiology of dipping is solid. The prognosis is messier.

A 2020 systematic review and meta-analysis pooled 10,438 adults with untreated high blood pressure across seven cohort studies. Dippers did have fewer cardiovascular events than non-dippers overall. But when the authors separated out the reverse dippers — the people whose pressure actually rises at night — the difference between plain non-dippers and dippers was no longer statistically significant. In other words, most of the risk signal was coming from the reverse-dipping group, not from everyone whose dip was merely shallow.

Why tell you that? Because it keeps you from two mistakes. The first is panic over a number you were never going to measure at home anyway. The second is dismissing sleep entirely when a headline gets walked back. The honest summary is this: sleep is firmly established as part of cardiovascular health — that is why it is on the checklist — while the finer question of exactly which overnight pattern predicts what is still being sorted out by people who do this for a living.¿Por qué le cuento esto?

"So should I take my blood pressure pill at night?"«¿Entonces debo tomar mi pastilla de la presión en la noche?»

This question comes up every single time, so let us settle it with the biggest trial that has asked it. The TIME trial, published in The Lancet in 2022, randomly assigned 21,104 adults in the UK to take their usual blood pressure medicines either in the morning or in the evening, and followed them for a median of 5.2 years. Major cardiovascular events occurred in 3.4 percent of the evening group and 3.7 percent of the morning group — not a statistically significant difference.

The practical reading: the best time to take your pills is the time you will actually remember every day. And the rule that never bends — never start, stop, skip, split or change the dose or timing of any prescription on your own, and never because of an article, including this one. That decision belongs to you and your prescriber, who knows your other medicines and conditions.

What is actually worth doing this weekQué vale la pena hacer esta semana

Not a 12-step routine. Four things, in the order I would pick them.

  1. Anchor the wake-up time, not the bedtime. It is the one you can control, and it drags the rest of the clock with it. Pick a time you can hold seven days a week — yes, including Saturday — and keep it within about an hour. If you are short on sleep, move bedtime earlier rather than moving the alarm later.
  2. Aim for the 7-to-9-hour window, from both sides. Long sleep is not a bonus: more than 9 hours is also associated with higher cardiometabolic risk in the AHA's statement. If you regularly need 10 hours and still feel tired, that is worth mentioning to your doctor rather than celebrating.
  3. Get morning light and move during the day. Light early and activity during waking hours are how the body's clock knows which way is up. Walking already earns its place for your circulation — it also helps your night.
  4. Stop treating loud snoring as a personality trait. Snoring with pauses in breathing, gasping, or waking unrefreshed despite enough hours can point to sleep apnea, which is both common and treatable, and which is one of the conditions most tightly tied to blood pressure. That is a conversation with your clinician, not a gadget purchase. I wrote a whole post on when snoring is worth checking.

And one thing to skip: do not buy an ambulatory monitor because of this article.

When to bring this to a doctorCuándo llevar esto a un médico

Make a regular appointment — not an emergency one — if any of these fit you:Pida una cita normal — no de emergencia — si algo de esto le corresponde:

🚨 Call 911 right away — do not wait, and do not drive yourself🚨 Llame al 911 de inmediato — no espere, y no maneje usted mismo

Call 911 (or your local emergency number) for chest pain or pressure, shortness of breath, back pain, numbness or weakness, trouble speaking, vision changes, a sudden severe headache, or confusion — at any hour, and whatever your blood pressure reading says. This is especially urgent if those symptoms come with a reading of 180 or higher on top and/or 120 or higher on the bottom. If you get a reading that high but feel completely well, sit quietly for five minutes and measure again; if it is still that high, contact your clinician the same day. A blunted overnight dip, by contrast, is never an emergency — it is a finding to discuss at an appointment.

Common questionsPreguntas frecuentes

Does blood pressure go down when you sleep?¿La presión arterial baja cuando uno duerme?

In most people, yes — typically by about 10 to 20 percent below daytime levels. That fall is called nocturnal dipping. A fall of less than 10 percent is called non-dipping, and a pressure that rises overnight is called reverse dipping. It is normal physiology, not something you succeed or fail at, and it is measured rather than felt.

Can my home monitor show my overnight pattern?¿Mi monitor de casa puede mostrar mi patrón nocturno?

No. A home cuff only captures the moments you are awake and using it. Sleeping blood pressure is measured with 24-hour ambulatory monitoring — a cuff worn for a full day and night that inflates automatically — and it is ordered by a clinician. Home readings are still very useful for the daytime picture and for sorting out office readings that do not match.No.

How many hours does my heart actually need?¿Cuántas horas necesita de verdad mi corazón?

The AHA set the ideal at 7 to 9 hours daily for adults when it added sleep as the eighth item in 2022, and the CDC recommends 7 or more hours for adults aged 18 to 60. Both ends matter: the AHA's 2025 statement notes that under 7 hours is associated with higher risk of atrial fibrillation and cardiometabolic syndrome, and that over 9 hours is also associated with higher risk of cardiometabolic syndrome, stroke and cardiovascular death.

It may be at least as important, which is newer than most heart advice. The AHA's April 2025 statement links greater consistency in sleep-wake timing to a 22 to 57 percent lower risk of cardiovascular death, and notes irregular timing was tied to higher type 2 diabetes risk even in people sleeping enough hours. A 2024 study of 60,977 adults found regularity predicted mortality risk better than duration. Duration still counts — regularity is simply a second lever most people were never told about.

Should I move my blood pressure pill to bedtime?¿Debo pasar mi pastilla de la presión a la hora de dormir?

Not on your own, and not because of an article. The TIME trial randomly assigned 21,104 UK adults to morning or evening dosing and followed them a median of 5.2 years; major cardiovascular events occurred in 3.4 percent of the evening group and 3.7 percent of the morning group, which was not a significant difference. The best time is the time you will reliably remember, and any change is a decision for you and your prescriber.

Dennis did not get a new prescription. He got a wake-up time. He kept 6:30 a.m. seven days a week for two months, moved bedtime earlier on the nights he could, and mentioned his snoring to his doctor, who took it seriously. His morning readings settled — not to zero drama, not overnight, but enough that the conversation changed from "why is this creeping up" to "let's keep watching this." That is what most real progress looks like.
