The short answer: usually neither single reading is "the right one" — the average of properly taken readings outside the office is what decides. A blood pressure number is a fact about you in that room, at that minute, not a fact about you in general, so one office reading was never built to settle the question. If your home numbers are reliably higher than the office's, that pattern has a name — masked hypertension — and it is the one most likely to be missed, because nothing in the visit flags it. Increasingly, the out-of-office one. A 2026 review of the 2025 AHA/ACC hypertension guideline describes out-of-office measurement as "a cornerstone of diagnosis and management," with both home monitoring and 24-hour ambulatory monitoring recommended for confirming a diagnosis — and home monitoring additionally recommended for ongoing management and medication adjustment. Position matters more than people expect, and there is a trial that quantifies it. The ARMS crossover randomised trial, published in JAMA Internal Medicine in 2024, compared arm positions in 133 adults. Against the correct position — arm supported on a desk at heart level — resting the hand in your lap raised systolic readings by about 3.9 mm Hg, and letting the arm hang unsupported at your side raised them by about 6.5 mm Hg. The authors concluded these common positions cause "substantial overestimation" and may lead to misdiagnosis. Beyond the single sitting, the approach the American Heart Association describes for home monitoring is a run of readings rather than a lone number: morning and evening over about a week, with the first day discarded (people are still learning the device and are a little nervous, so day one tends to run high), and the rest averaged. I think we spent decades emphasising the wrong one of these two, and the guidelines are quietly correcting course. "White coat" entered ordinary speech as a reassurance — it's just nerves, your real pressure is fine. Meanwhile the pattern that runs the other way, the silent one, went largely unscreened while carrying risk that the evidence puts close to sustained hypertension. We built a folk vocabulary around the harmless-sounding version and never gave the dangerous version a household name.

Watch the direction of travel in the guidelines themselves. In 2017, out-of-office measurement was essentially the confirmation step — something you reached for after a clinic number looked off. By 2025 it is described as a cornerstone of both diagnosis and ongoing management, including medication adjustment. My read is that this trajectory ends with out-of-office measurement becoming the primary way blood pressure is defined, and the office reading demoted to what it has arguably always been: a screening trigger, not a verdict.

Which leaves a genuinely strange situation, and it is the part I most want doctors to sit with. If that is where we are heading, then the most decisive blood-pressure measurement many people will ever have is one they buy themselves, run unsupervised, and are rarely taught to perform properly — while the device most of them would instinctively reach for, the watch on their wrist, is explicitly not recommended for the job. That is an argument for clinicians teaching the technique deliberately rather than shrugging at home numbers, and for patients feeling entitled to bring a week's log to an appointment and have it taken seriously.

To be very clear about what this is: a perspective, not a guideline, and not a reason to doubt your own doctor's plan, skip an appointment, or change a medication. It is a reason to ask one better question at your next visit.

The word AND is doing real work there. A very high number on its own, with no symptoms, is an urgent call to your doctor's office — not a 911 call. Both halves of that sentence matter, and consumer health content drops one of them constantly.

It might be — but check technique before you blame the device, because position alone can move a systolic reading by several points. Take it to your next appointment and ask the staff to compare it against theirs on the same arm, one after the other. That takes two minutes and settles the question properly. The usual answer is not to stop, but to check on a schedule your doctor agrees to, write the numbers down, and look only at the average. Yes, and a small difference is common. It is worth having both arms checked at least once, because a persistent, larger gap between arms is itself something a doctor may want to look into.
