The right side takes back the blood that has already done its job and sends it to the lungs for a fresh load of oxygen. The trip down is powered by that push. Arteries are thick, muscular, high-pressure tubes built to absorb the shock of every heartbeat — which is why you can feel a pulse in an artery and not in a vein. A little fluid leaks out here on purpose. Every drop returning from your leg has to cross the pelvis through the iliac veins to reach the vena cava. It is a tight junction: the left common iliac vein passes underneath the right common iliac artery, pinned against the spine. Pairs of thin, flexible flaps along the inside of a vein. Blood moving upward pushes them open; blood trying to fall back fills them and snaps them shut. They are most numerous below the knee, where the column of blood pressing down is tallest. Each squeeze of the calf sends a slug of blood upward. The flaps swing open to let it through, then fill from above and seal. The blood that just went up cannot come back down, so the next squeeze starts from a higher point. Blood still goes up with each squeeze — but some of it slides straight back down, and the column below never gets shorter. Doctors call that backward slide reflux, and the steady high pressure it leaves behind is what the term chronic venous insufficiency describes.
