The billboard said: "Varicose veins? Gone on your lunch break." Marisol drove past it twice a day for a year. What it didn't tell her is the one thing that decides whether any vein treatment works — and it has nothing to do with lasers, needles or glue.

Last week, in the shingles shot and your heart, I promised we'd come back to your legs. Here it is: every common varicose vein treatment in plain English — what each one actually does, who it's for, what the weeks after look like — and the eight questions I'd want you to ask before you choose one.

Don't treat the bulge before you find the leak.

Marisol and the three different answersMarisol y las tres respuestas distintas

Marisol is 49 and drives a school bus. By the afternoon route, the ropey veins behind her right knee feel heavy and achy, like her leg is "full." She asked around. Her coworker said, "Get the injections, that's what I did." Her cousin said, "No, get the laser." Her aunt said, "In my day they pulled the whole vein out. I still have the scars."Marisol tiene 49 años y maneja un autobús escolar.

"Doctor, three people, three answers. Which one is the right one?"«Doctor, tres personas, tres respuestas. ¿Cuál es la correcta?»

My answer surprised her: all three can be right — for different veins. The real question isn't "which treatment?" It's "which vein is the problem?" And you usually can't answer that by looking in the mirror.

The leak and the bulgeLa fuga y el bulto

Your leg veins carry blood uphill, back to your heart. Tiny one-way valves inside them work like little doors that only swing up. When some of those doors stop closing well, blood slips backward and pools. The vein stretches, twists and bulges. That's a varicose vein.

Here's what most people don't know. The twisty vein you can see is often a branch. The leak feeding it may sit higher up, in a bigger "trunk" vein that runs a little deeper under the skin, where you can't see it. Think of a stain on your ceiling. You can paint over the stain — and it comes right back — until someone finds the leaky pipe in the attic.

That's why the first step isn't a procedure. It's a map: a duplex ultrasound, a painless scan that shows which way blood flows in each vein. A large review behind the U.S. vein guidelines called it the gold-standard test, and the guidelines from the Society for Vascular Surgery and two vein societies call for it in people with varicose veins, done standing whenever possible — because leaky valves show themselves best when gravity is pulling.

The leak and the bulge: a hidden leaky trunk vein feeding a visible bulging branch, and which treatments fit each La fuga y el bulto: una vena tronco escondida que gotea y alimenta una rama abultada visible, y qué tratamientos sirven para cada una On the left, an outline of a leg. A long teal trunk vein runs up the inner leg with an open, leaky valve near the top marked with a coral drop: the leak, often hidden. Lower down, a twisty coral branch vein bulges near the calf: the bulge you see. A dashed magnifier labeled duplex ultrasound, standing, covers the trunk. On the right, two columns. Trunk leak: heat (radiofrequency or laser) or glue through a thin tube; sometimes foam injection. Bulging branches: injections for smaller veins, or lifting out through tiny cuts. Under both: stockings and walking help symptoms either way. The leak La fuga often hidden in a a menudo escondida trunk vein en una vena tronco Map it: duplex Mapa: ultrasonido ultrasound, standing dúplex, de pie The bulge El bulto the branch you see la rama que usted ve If the trunk is leaking Si el tronco gotea • Heat: radiofrequency or laser • Calor: radiofrecuencia o láser • Medical glue • Pegamento médico • Sometimes foam injection • A veces, espuma inyectada thin tube, numbing medicine, awake tubo delgado, anestesia local, despierto For the bulging branches Para las ramas abultadas • Injections, for smaller veins • Inyecciones, para venas menores • Lift out through tiny cuts • Sacarlas por cortes diminutos same day as the trunk, or weeks later el mismo día que el tronco, o después Either way: stockings + walking En cualquier caso: medias + caminar help symptoms; they don't close a leak alivian síntomas; no cierran la fuga Illustration, not to scale. Your clinician matches the treatment to your own ultrasound map. Ilustración, no a escala.
The vein you see is often the branch. The leak may be higher, in a trunk vein you can't see — so the map comes before the treatment.

Almost every modern treatment works the same way at heart: it closes a vein that is carrying blood the wrong way. People panic at that — "Don't I need that vein?" Here's the reassuring part. A leaky vein is like a road that sends traffic backward. Close it, and the blood takes the other, healthy roads home. As MedlinePlus puts it for heat treatment, the vein is closed off and disappears over time.

The toolbox, in plain EnglishLa caja de herramientas, en lenguaje sencillo

1. Squeeze and move: compression stockings, walking, legs up. For many people, this is the first step. Mayo Clinic says wearing compression stockings all day is often the first approach to try. MedlinePlus notes self-care can relieve achiness and help keep varicose veins from getting worse.1.

What it can't do: close a leak. Stockings help while you wear them. Ask first if you have artery problems in your legs (PAD), diabetes, or numb feet — compression isn't safe for everyone (more in who should not wear compression socks).Lo que no puede hacer: cerrar una fuga.

2. The injection: sclerotherapy (and foam). A solution is injected into the vein, which hardens and fades. MedlinePlus lists it for a small number of varicose veins; it's also the classic fix for spider veins. A foam version, guided by ultrasound, is also used for some larger veins, including trunk veins; it was among the methods compared in the 2022 review behind the U.S. guidelines.2. UU.

What to expect (Cleveland Clinic):Qué esperar (Cleveland Clinic):

  • You may need more than one session, usually two to three months apart.
  • Spider veins often fade in three to six weeks. Larger veins can take three to four months.
  • Brown marks can show up. They usually fade within a few months.Pueden salir manchas cafés. Suelen desaparecer en unos meses.
  • Stockings or wraps for three to seven days. Walking helps.Medias o vendas de tres a siete días. Caminar ayuda.

3. The heat: radiofrequency or laser ablation. For larger, longer veins, MedlinePlus says your provider may suggest a procedure using a laser or radiofrequency, done in the office. A thin tube (catheter) goes into the vein through a needle stick; heat closes the vein from the inside.3.

What to expect (MedlinePlus):Qué esperar (MedlinePlus):

  • Numbing medicine. You stay awake.Anestesia local. Usted se queda despierto.
  • Your leg is wrapped for 2 to 3 days.La pierna queda vendada de 2 a 3 días.
  • Normal activities within 1 to 2 days.Actividades normales en 1 a 2 días.
  • Compression stockings during the day for about a week.
  • Often an ultrasound a few days later, to make sure the vein is sealed.

4. The glue: closure with medical adhesive. Also through a thin tube, but instead of heat, a medical adhesive seals the vein shut. The U.S. guidelines weigh heat versus non-heat methods as two real options for leaky trunk veins — not one "new and better" and one "old."4.

5. The lift-out: ambulatory phlebectomy. Tiny cuts are made next to a bulging branch, and the vein is removed through one of the cuts. It's often used for the twisty branches you can see — sometimes the same day the trunk is closed, sometimes weeks later. Doing it together or in stages is a real choice the guidelines address; ask which your clinic recommends, and why.5.

6. Your aunt's surgery: ligation and stripping. The vein is tied off and pulled out through incisions. It still exists, but radiofrequency, laser and injections have largely replaced it. If your picture of vein treatment comes from a relative's 1990s scars, today's office procedures are a different experience.6.

Remember the first idea: these tools aren't rivals. They're matched to which vein is leaking and how big it is. That's why Marisol's three advisers could all be right — for their own legs.

My perspective as a physicianMi perspectiva como médico

Vein ads sell the machine: the laser, the glue, the "lunch-break fix." I'd rather you shop for the map — a careful standing ultrasound, read by someone who explains it to you — and for the plan that comes after it. A great tool aimed at the wrong vein is still the wrong treatment.

And one more honest thing: leaky veins are a tendency, not a single broken part. Cleveland Clinic notes varicose veins can return even after effective treatment, and new ones can appear elsewhere. So I think of treatment as a chapter, not the last page — walking, weight and follow-up still matter. This is my labeled perspective, not a guideline; talk it through with your own clinician.Y una cosa más, con honestidad: las venas que gotean son una tendencia, no una sola pieza rota.

Comfort, looks, and who paysComodidad, apariencia y quién paga

Be honest with yourself about your goal, because it changes the conversation. Aching, heaviness, swelling, itching or skin changes are symptoms. Wanting smoother-looking legs is a fine goal too — but Mayo Clinic notes that treatment done only for looks is called cosmetic, and insurance might not cover it. Call your insurer before you book, not after.

And if you have no symptoms and no skin changes, waiting is a real choice to discuss with your clinician. MedlinePlus notes varicose veins tend to get worse over time, so "waiting" works best as "watching": self-care now, and a check if anything changes.Y si no tiene síntomas ni cambios en la piel, esperar es una opción real para conversar con su médico.

The 8 questions to ask before you chooseLas 8 preguntas que debe hacer antes de elegir

Screenshot this list, or print it. A good vein clinic will be glad you asked.

  1. "Was my ultrasound done standing — and which vein is actually leaking?" Ask them to show you the trunk versus the branches.«¿Me hicieron el ultrasonido de pie — y cuál vena es la que gotea?»
  2. "What do you recommend for my veins, and why that one over the others?" You want a reason tied to your map, not to the machine they own.«¿Qué me recomienda para mis venas, y por qué esa opción y no las otras?»
  3. "Is this for my symptoms, for looks, or both — and will my insurance see it the same way?"«¿Esto es por mis síntomas, por apariencia o por ambas — y mi seguro lo verá igual?»
  4. "How many sessions do you expect, and what is the full cost?" Include follow-up visits, ultrasounds and stockings.«¿Cuántas sesiones calcula, y cuál es el costo total?» Incluya consultas de seguimiento, ultrasonidos y medias.
  5. "Will you treat the bulging branches the same day, or later?"«¿Va a tratar las ramas abultadas el mismo día, o después?»
  6. "What will the next few weeks feel like — and what would make you want to see me right away?" (Here's my guide to what's normal after vein treatment.)«¿Cómo me voy a sentir en las próximas semanas — y qué le haría querer verme de inmediato?»
  7. "Will I have a follow-up ultrasound, and when?"«¿Me harán un ultrasonido de control, y cuándo?»
  8. "What happens if I wait?" An honest answer to this one tells you a lot about a clinic.«¿Qué pasa si espero?»

Nervous about asking? Rehearse it first in the Visit Simulator. And if the answers don't add up, getting a second opinion is normal, not rude.¿Le da nervios preguntar?

Marisol's standing ultrasound found the leak exactly where she couldn't see it: a trunk vein running up the inside of her thigh, feeding the bulging branches behind her knee. "So if I'd only had the injections on the bumps," she said, "they'd have come back?" Quite possibly — the leak would still be feeding them.

She wore stockings for a few months while she checked her coverage and asked all eight questions. Then she had the trunk vein closed with heat in the office, and the largest branches lifted out a few weeks later. She was back on her bus route two days after the first procedure, with some bruising and a tender cord along her thigh that looked worse before it looked better — a normal part of healing for many people. By spring, the afternoon heaviness was gone. She still walks on her break. "The veins were the chapter," she told me. "The walking is the book."Usó medias por unos meses mientras revisaba su cobertura y hacía las ocho preguntas.

Your three-step plan this weekSu plan de tres pasos esta semana

  1. Name your goal. Write one line: "My veins bother me because…" Comfort? Looks? Swelling? Skin changes? That sentence steers the whole visit. ¿Comodidad? ¿Apariencia? ¿Hinchazón? ¿Cambios en la piel? Esa frase guía toda la consulta.
  2. Ask for the map first. Before agreeing to any procedure, ask whether a duplex ultrasound — done standing — has shown which vein is leaking.
  3. Bring the 8 questions. Screenshot the list above, and practice saying them out loud in the Visit Simulator.

One more time, because it's the whole point: injections, heat and glue are all good tools. Which one is right depends on one thing first — finding the leak, not just treating the bulge.

What is the best treatment for varicose veins?¿Cuál es el mejor tratamiento para las várices?

There is no single best treatment for everyone. The right choice depends on which veins are leaking, how big they are, and whether your goal is comfort, looks, or both. That is why a duplex ultrasound, done standing whenever possible, comes first: U.S. vein guidelines (SVS/AVF/AVLS) call for it before treatment. Common options are compression stockings and walking, sclerotherapy injections for smaller veins, heat ablation (radiofrequency or laser) for larger veins, non-heat closure with medical adhesive, and removing bulging branches through tiny cuts (ambulatory phlebectomy). Many people get a combination.

Is varicose vein treatment painful, and how long is recovery?¿Duele el tratamiento de várices, y cuánto dura la recuperación?

Most modern treatments are done in the office. For heat ablation, MedlinePlus says you get local anesthesia to numb the leg and stay awake, the leg is wrapped for 2 to 3 days, most people return to normal activities within 1 to 2 days, and compression stockings are worn during the day for about a week. After sclerotherapy, Cleveland Clinic says support stockings or wraps are worn for 3 to 7 days and walking helps recovery. Some bruising, tenderness or a firm cord along the treated vein can happen; ask your clinician what is normal for your procedure.

Will insurance pay for varicose vein treatment?¿El seguro paga el tratamiento de várices?

It depends on your plan and your reason. Mayo Clinic notes that treatment done only to make the legs look better is considered cosmetic, and insurance might not cover it. Plans have their own rules for covering treatment of symptoms, so ask your insurer what yours requires. Ask your insurer before you book, and ask the clinic for the full cost, including any follow-up ultrasound.

Can varicose veins come back after treatment?¿Pueden volver las várices después del tratamiento?

Yes. Cleveland Clinic notes that varicose veins can return even after effective treatment, and MedlinePlus lists the return of the varicose vein over time among the risks. A treated vein that stays closed can still be followed by new veins in other places, because the tendency to develop leaky veins remains. Walking, staying active, and keeping a healthy weight can help, and some people need touch-up treatment later.Sí.

When is a varicose vein an emergency?¿Cuándo es una emergencia una várice?

Get same-day medical care for sudden swelling, pain, warmth or redness in one leg, which can be a blood clot. Call 911 for chest pain, sudden shortness of breath, or coughing up blood, which can mean a clot has reached the lungs. If a varicose vein bleeds, lie down, raise the leg, and press firmly on the spot; call 911 if the bleeding is heavy or will not stop. See your clinician for leg sores that do not heal.

It does not recommend or rank any clinic, device or brand, and no procedure is right for everyone; recovery details vary by clinic and procedure. The view that treatment is "a chapter, not the last page" is the author's labeled perspective, not a guideline. Check with your clinician before using compression if you have artery disease, diabetes or numb feet. Never start, stop or change any medicine on your own. Call 911 (or your local emergency number) right away for chest pain, sudden shortness of breath, coughing up blood, or heavy bleeding that will not stop. Get same-day care for sudden swelling, pain, warmth or redness in one leg. Marisol is a composite drawn from many years of practice; details have been changed and no one described here is a real individual.
