The short answer: usually not on your own — because for most people the normal number is the medicine working, so stopping tends to send it back up. But "never" is not the honest answer either. Doctors do deliberately reduce and sometimes stop these medicines, there are randomized trials on exactly that, and the practice has a name: deprescribing. What it never has is a solo start. Some blood pressure medicines are genuinely dangerous to stop abruptly, and untreated high pressure does its damage in silence. So the useful question is not "can I stop?" It is "is reducing reasonable for me, and how would we watch it?" In 2025 a Cochrane review — the most conservative kind of evidence summary there is — pooled six randomized trials and 1,073 people who either continued or stopped their blood pressure medicine. OPTiMISE is the one to know. Some blood pressure medicines cause a dangerous rebound when they are stopped abruptly — not simply a return to your old number, but an overshoot above it, sometimes within a day or two. The clearest example is in the FDA prescribing information for clonidine, which states that sudden cessation has produced "nervousness, agitation, headache, and tremor accompanied or followed by a rapid rise in blood pressure," and — I am quoting the label, not dramatizing — "rare instances of hypertensive encephalopathy, cerebrovascular accidents and death have been reported after clonidine withdrawal." That same label instructs the prescriber to reduce the dose gradually over 2 to 4 days. Names blur. It asks for monitoring rather than trying to escape it. Here is my read, labelled as a read. If your doctor's answer to this question has ever felt vague, I want to offer you a more generous explanation than the one most people reach for. It is vague because the evidence is. Medicine has spent decades and enormous resources studying when to start blood pressure treatment, which drug, and to what target — that is why the 2025 guideline can tell a clinician to treat at 130/80 above a specific ten-year risk, and to try three to six months of lifestyle changes first below it. Now look at the other direction: the 2025 Cochrane review on stopping found six randomized trials, 1,073 people, and rated the certainty of every outcome as low to very low. And that has a consequence I would like more patients to hear, because it flips the usual feeling of this conversation. When evidence is thin, the decision stops being a fact to be looked up and becomes a genuine judgment call — which means your priorities legitimately belong in it. How much the pill bothers you, what a smaller pill count is worth to you, how you weigh a small extra risk against a daily annoyance: those are not noise to be overruled by data. In a low-certainty decision they are part of the data. Almost always no — and the reason is the single most useful idea in this whole article. For most people the normal reading is not evidence the condition left; it is evidence the medicine is working. High blood pressure is not an infection that a course of pills clears. It is closer to a thermostat holding a room at a comfortable temperature in January: the comfortable temperature is not proof that winter ended. That is why the 2025 Cochrane review found that people who stopped had roughly three times the odds of needing to restart, with systolic pressure rising about 9.75 mm Hg on average. There is one real exception worth naming: when the thing that raised your pressure genuinely changed — significant weight loss, much less salt, treated sleep apnea, less alcohol, a medicine removed that was raising it — the underlying situation can be different, and that is exactly the conversation to have with your doctor.Casi siempre no — y la razón es la idea más útil de todo este artículo. For some medicines, yes — genuinely dangerous, and this is not a scare tactic. Clonidine's FDA prescribing information states that sudden cessation has caused nervousness, agitation, headache and tremor followed by a rapid rise in blood pressure, and that rare instances of hypertensive encephalopathy, cerebrovascular accidents and death have been reported after clonidine withdrawal. The same label instructs prescribers to reduce the dose gradually over 2 to 4 days. Beta blockers carry their own withdrawal concerns, particularly in people with heart disease. Other classes are far less dramatic. The practical problem is that most people cannot tell from the pill bottle which category they are holding — which is precisely why the decision belongs with the prescriber or pharmacist, who can look. Never stop, skip, halve or space out a blood pressure medicine on your own.Con algunos medicamentos, sí — genuinamente peligroso, y esto no es para asustar. Yes, and the answer surprises most people. The OPTiMISE randomized trial deliberately removed one antihypertensive from 569 patients aged 80 and older whose systolic pressure was under 150 mm Hg. At 12 weeks, 86.4% of the reduction group and 87.7% of the usual-care group were still under 150 — noninferior — with average systolic pressure 3.4 mm Hg higher in the reduction group. Four years later, all-cause hospitalisation or death showed a hazard ratio of 0.93 (95% CI 0.76 to 1.12), and the authors concluded that deprescribing an antihypertensive medication might be safe in older patients with controlled blood pressure and multiple long-term conditions. The honest caveat: that trial studied removing one pill from people taking several, under supervision, not stopping treatment altogether — and the 2025 Cochrane review rates the overall evidence as low to very low certainty.Sí, y la respuesta sorprende a casi todos. For some people, yes — and there is a real trial rather than a slogan behind that. TONE randomized 975 adults aged 60 to 80 whose pressure was controlled on a single medicine, gave them sodium reduction, weight loss, both or usual care, then attempted to withdraw the medicine after three months. Thirty months later, 37.8% of the sodium-reduction group were still free of blood pressure medicine and free of cardiovascular events, versus 24.4% of those not assigned to it; for weight loss the figures were 39% versus 26%. Read those numbers honestly in both directions. Lifestyle change roughly doubled the odds of staying off — and even in the best group, most people still needed medicine again. That is not failure; it is the actual shape of the result, and it is why this is attempted with monitoring rather than announced as a plan.En algunas personas, sí — y detrás de eso hay un ensayo real, no un eslogan. Use the word "reduce" rather than "stop", bring data rather than a wish, and ask for a plan rather than permission. Something like: "My home readings have averaged about 118 over 72 for the last month, I've lost fourteen pounds, and I'd like to know whether reducing one of my medicines is reasonable — and if we tried, how we would monitor it and when we would put it back." That single sentence does four things a request to stop does not: it supplies evidence, it names a smaller and more achievable step, it invites monitoring instead of avoiding it, and it pre-agrees that going back on is a planned outcome rather than a defeat. Bring your home readings, your full medicine list including anything over the counter, and any side effect that is bothering you — side effects are a legitimate reason to revisit a regimen, and your doctor would rather hear about them than discover an untaken prescription.Use la palabra «reducir» en vez de «dejar», lleve datos en vez de un deseo, y pida un plan en vez de permiso.
