Your Ankles Are Swollen — So You Drank Less Water. Here's Why That Backfires.

A woman sat across from me last summer and described, very calmly, a plan she had been running for two months. Her ankles puffed up every afternoon. So she had cut her drinking down to almost nothing — "so I don't hold it," she said.

It is one of the most sensible-sounding wrong ideas in all of medicine. Water goes in, water shows up in the ankles — of course you would turn down the tap. Nearly everyone I meet with swollen legs has tried it at least once.

Her ankles were exactly the same size as when she started. Something else had changed, though — and it was the thing she had not connected to any of this.

You can be swollen and dehydrated at the same time. They are two different problems.

Marisol is 61 and runs a middle-school lunchroom. She is on her feet from six in the morning, on a concrete floor, in a Texas August. By three in the afternoon her socks left a ring above each ankle that she could still see at bedtime.

She did not come to see me about swelling. She came because of the headaches — every afternoon, like clockwork — and because twice that month she had stood up from a chair and had to grab the counter while the room went grey at the edges. She was also, she mentioned almost as an afterthought, tired in a way that sleep was not fixing.

Her ankles, she said, were "just a summer thing." She had them handled.Los tobillos, dijo, eran «cosa del verano». Eso ya lo tenía resuelto.

"I stopped drinking so much. Doesn't help much, but at least it's not making it worse."«Dejé de tomar tanto.

I asked what colour her urine was. She laughed, and then she stopped laughing, because she knew the answer and she had known it for a while.

Before any of the explaining: most everyday ankle swelling is boring, both-sided, worse by evening and gone by morning. Some swelling is not. Please read this box before you read the rest of the article — none of it is behind anything. Two accounts, not oneDos cuentas, no una

Here is the misunderstanding, and once you see it you cannot unsee it. People picture the body as one tank of water. Add water, tank fills, ankles swell. Remove water, tank empties, ankles shrink. Simple, tidy, and wrong. Sencillo, ordenado y falso.

The body keeps water in more than one place. Some of it is inside your blood vessels — the fluid actually circulating, carrying oxygen, holding up your blood pressure. Some of it is out in the tissue between the cells, in the soft padding of your ankle. Swelling — the medical word is edema, and MedlinePlus defines it as swelling caused by fluid in the body's tissues — is water in the second place.

That water did not fall from the sky. It came out of the pipes. And that is the whole thing: drinking less does not pull it back in. It just leaves less in the pipes.

Where the water actually is Dónde está el agua en realidad MORNING MAÑANA tissue tejido bloodstream — full torrente sanguíneo — lleno No ring. Sin marca. LATE AFTERNOON POR LA TARDE tissue — waterlogged tejido — encharcado Sock ring. Pipes running low. Marca del calcetín. Same body. Same water. It changed compartments — it did not appear out of your glass.
Original illustration · Dr. Schamma Explains — edema is fluid that has left the vessels and settled in the tissue, which is why a swollen ankle and an under-filled bloodstream can happen in the same leg on the same afternoon. Compartments and definitions as described by MedlinePlus.

This is why Marisol's plan produced exactly the result it produced. Her ankles never changed, because she was not treating the reason fluid was leaving her vessels — standing still on concrete for nine hours in the heat, with gravity doing what gravity does. Meanwhile the compartment she was successfully draining was the one holding up her blood pressure. Hence the grey edges when she stood up. Hence the headaches. Hence the tiredness sleep would not touch.

You do not have to take my word for any of this. MedlinePlus lists what providers recommend for managing edema, and it is a short list:No tiene que creerme a mí nada de esto.

Read it again and notice what is missing. Salt is on the list. Water is not. That is not an oversight — it is the mechanism. Salt is the thing that decides how much fluid your body holds onto and where it parks; if you want the long version of where sodium actually hides, I wrote it here: where sodium really hides in your food. Fluid restriction does appear in medicine, and it matters enormously — but it appears as a prescription for a specific person with a specific condition, arrived at with their weights and their labs in front of the doctor. It is not a home remedy for puffy ankles at the end of a long shift.Léala otra vez y note lo que falta.

Two dials, and most people only ever read oneDos perillas, y casi nadie lee más que una

Once you accept that swelling and hydration are separate accounts, you need a way to check them separately. Here is the version I actually teach in the room. It takes five seconds and costs nothing. Toma cinco segundos y no cuesta nada.

Two dials — read them separately Dos perillas — léalas por separado ANKLES PUFFY TOBILLOS HINCHADOS ANKLES NORMAL TOBILLOS NORMALES Urine dark yellow Orina amarilla oscura Urine pale or straw Orina clara o pajiza Swollen AND dry Hinchado Y seco The common August one. Both dials are wrong, in opposite directions. Drink toward pale — and move, elevate, check salt. Just dry Solo seco Nothing to do with your veins. Very ordinary. Drink. Don't wait until you feel thirsty. Tome agua. No espere a tener sed. Hydrated, but pooling Hidratado, pero se acumula Water is not the lever here. Look at salt, long standing, long sitting, heat, and any new medication. If it is new, get it checked. Where you want to be Keep doing what you're doing. New swelling in ONE leg is not a dial reading. Neither is swelling with breathlessness. Those are same-day calls.
Original illustration · Dr. Schamma Explains — swelling and hydration move independently, so a single glance at your ankles tells you nothing about the other dial. Urine-colour guidance as described by Cleveland Clinic; edema management as listed by MedlinePlus. This is a way to think, not a diagnosis.

Cleveland Clinic's rule for the second dial is about as simple as medicine gets: if your urine is clear, pale or straw-coloured, it's OK — if it's darker than that, keep drinking. And they add the sentence I wish were printed on every water bottle: "If you feel thirsty, you're already dehydrated." Thirst is not an early warning light. It is the light that comes on after the fact.

That matters most for exactly the people most likely to be cutting fluids for their ankles. Cleveland Clinic notes that older adults are at higher risk of dehydration because they carry less water in their bodies and can't tell as easily when they're thirsty. Sit with that for a second. The group most likely to be told "watch your fluids" is also the group whose thirst alarm is quietest. That combination is how a person ends up in my office in August with a headache, grey vision on standing, and ankles that never got any smaller.

"Eight glasses a day" is a number with no person attached to it — and it is, oddly, both too rigid and too small. Here is the real figure. Mayo Clinic cites the U.S. National Academies of Sciences, Engineering, and Medicine: about 15.5 cups (3.7 L) of fluids a day for men and about 11.5 cups (2.7 L) for women. But read the fine print, because it is the whole story — that total covers water, other beverages and food, with about 20% normally coming from food. So the famous eight glasses is neither the target nor the unit.

My honest opinion, offered as opinion: stop counting glasses and start reading the two dials. A number you have to remember is a number you will abandon by Thursday. A colour you can check in four seconds, twice a day, is a habit. Cleveland Clinic's own framing supports the individual approach — the right amount depends on your weight, age, activity and climate, and is a question for your provider rather than a universal quota.

The part that may become tomorrow's normalLa parte que quizá sea lo normal de mañana

Now something more speculative, and I am going to label it clearly, because the line between "interesting" and "proven" is where a lot of health writing quietly cheats.

In 2023 the National Institutes of Health published a study in eBioMedicine that followed 11,255 adults over 30 years. The researchers used serum sodium — a routine blood value that rises as fluid intake falls — as a stand-in for long-term hydration. Adults whose sodium sat at the higher end of the normal range were more likely to develop chronic conditions, more likely to show signs of advanced biological aging, and more likely to die at a younger age than adults sitting in the middle of that same normal range.

What that does not mean. This is an observational study. It shows an association, not proof that drinking more water slows aging. Serum sodium is an imperfect stand-in for hydration — plenty of things nudge it. And people who are chronically under-hydrated may differ from their neighbours in a dozen other ways the study cannot fully separate out. Nobody should read that paragraph and start forcing fluids.

What I find genuinely worth thinking about is the shape of the finding, because it is a shape this site keeps running into: everyone in that study was "normal." Every one of those sodium values would have come back on a lab report with no flag on it, no asterisk, no phone call. And yet the top of normal behaved differently from the middle of normal. I wrote a whole article about that pattern — what the "normal range" really means — and hydration may turn out to be one of its clearest examples. File it under food for thought, not under instructions.

Common questionsPreguntas frecuentes

Should I drink less water because my ankles swell?¿Debo tomar menos agua porque se me hinchan los tobillos?

Not on your own, and usually not at all. Look at what MedlinePlus actually lists for managing edema: keeping the legs elevated when sitting, wearing support stockings, limiting salt, and taking diuretics if a provider prescribes them. Limiting water is not on that list. Salt is. The one situation where fluid restriction is correct is when your own doctor has set a limit for you — commonly in heart failure, kidney disease, dialysis or liver disease, or alongside certain medications. That instruction is personal, it is based on your tests, and it outranks anything you read here.

How much water do I actually need in a day?¿Cuánta agua necesito realmente al día?

There is a published figure, and it is bigger and looser than most people think. Mayo Clinic cites the U.S. National Academies of Sciences, Engineering, and Medicine: about 15.5 cups (3.7 L) of fluids a day for men and about 11.5 cups (2.7 L) for women. The crucial detail is what counts — that total includes water, other beverages and food, with roughly 20% of it normally coming from food. So it is not a plain-water quota. Cleveland Clinic makes the other half of the point: the right amount depends on your weight, age, activity and climate, and is worth confirming with your own provider. Two signals beat any number — urine that is pale, and not waiting until you feel thirsty.

Do coffee and tea count toward my fluids?¿El café y el té cuentan como líquidos?

The National Academies figure that Mayo Clinic cites is a total-fluid figure covering water, other beverages and food — not a plain-water-only quota. That said, MedlinePlus's own advice for preventing dehydration is to drink water and to avoid sugary and caffeinated drinks, so it is not a wash either. The practical version I give people: let plain water be the default and let coffee or tea be a pleasure rather than the plan. If you are drinking four coffees and no water and your urine is dark, the answer is not a debate about caffeine — it is a glass of water.

Can you drink too much water?¿Se puede tomar demasiada agua?

Yes, though for most healthy adults it is uncommon. Cleveland Clinic explains that when the kidneys are working as they should, drinking too much water almost never drops blood sodium too low — but that taking in an excessive amount can overwhelm the kidneys' ability to clear it, which is why low sodium turns up in endurance events like marathons, where people drink heavily without replacing electrolytes. Heart, liver, kidney and brain conditions, hormone problems and some medications also cause it. This is exactly why nothing here tells anyone to force fluids: the goal is pale urine and not being thirsty, not a race to the bottom of a jug.Sí, aunque en la mayoría de los adultos sanos es poco común.

Why do my ankles swell more in summer and on planes?¿Por qué se me hinchan más los tobillos en verano y en los aviones?

MedlinePlus lists standing or walking in warm weather among the ordinary causes of edema, and heat and long stillness are the same story from two directions: warmth relaxes the vessels while the calf pump — the muscle action that pushes blood back up out of the legs — is either idle in a seat or working against gravity all day. Gravity finishes the job. That kind of swelling is usually both-sided, arrives late in the day and is gone by morning. What is not ordinary: swelling in one leg only, especially with pain, warmth or redness, or any swelling that comes with breathlessness. Those are not a heat problem and need to be seen.

What happened to MarisolQué pasó con Marisol

We checked her over, because "it's just a summer thing" is a conclusion, not a finding, and it is not mine to accept without looking. Her swelling was on both sides, gone by morning, with no breathlessness — the ordinary kind. What was not ordinary was everything she had built on top of it.La revisamos, porque «es cosa del verano» es una conclusión, no un hallazgo, y no me corresponde aceptarla sin mirar.

She kept a bottle at the serving line. She walked a lap of the kitchen every hour. She did her heel raises while the ovens heated. She read one label — the ranch dressing — and put it back on the shelf, which is a very good afternoon's work. And at the end of a shift she put her feet up on two pillows for twenty minutes instead of sitting with them down.

The headaches went first — within about a week. The grey edges when she stood up stopped. Her ankles improved too, but slowly, and honestly they still puff a little by Friday in August, because she still works nine hours on a concrete floor and I cannot change that. She did not need new ankles. She needed the fluid to be in the right compartment, and her calves switched on.Los dolores de cabeza se fueron primero — como en una semana. No necesitaba tobillos nuevos.

The thing I keep coming back to is what her plan cost her. She spent two months making herself dizzy, headachy and exhausted — and it never once touched the problem she was trying to solve. That is not a failure of willpower or intelligence. It is what happens when a sensible-sounding idea about the body turns out to be about the wrong compartment.

For educational purposes only. This article is general health education, not medical advice, diagnosis, or treatment, and it does not create a doctor–patient relationship or replace your own doctor. Swelling in the legs has many possible causes — including heart, kidney, liver, vein and lymphatic conditions, pregnancy and medications — and only a clinician who can examine you and review your history, medications and test results can tell you which applies to you. Always consult your own doctor, and never start, stop or change any medication, diuretic or supplement on your own. If your doctor has set a daily fluid limit for you, follow their limit — nothing here overrides it, and do not increase your fluid intake based on this article. Research figures cited describe specific study populations and do not predict what will happen in any individual case; the hydration and aging study described here is observational and does not establish cause and effect. If you have sudden shortness of breath, chest pain or pressure, coughing up blood, fainting, new confusion, or new swelling in one leg with pain, warmth or redness, seek care immediately — call 911 (or your local emergency number) for breathing or chest symptoms, and do not drive yourself. Marisol is a composite drawn from many years of practice; details have been changed and no one described here is a real individual.
